To authorize monthly donations from a credit card or bank account, please fill out our monthly donor form and send it to us by mail or by fax.
CHANGING YOUR DONOR INFORMATION
To change your monthly donation amount or update your credit card, please contact info@stephenlewisfoundation.org or call 416-533-9292 or toll-free at 1-888-203-9990.
To notify the Foundation of a change to your banking information, please mail us a voided cheque along with your name and address.
TAX RECEIPTS FOR MONTHLY DONATIONS
If you are making monthly donations through your bank account or credit card, your annual receipt will be sent out in January for the previous tax year.
